CYP450 test source

Just found a post on Kevin, M.D. about CYP450 testing. My elderly mother is a poor metabolizer who is in cronic pain and needs help asap. I would like to know where to obtain the AmpliChip test for her.

Anyone with knowledge about possible pain medication for elderly (85) person (known to be a slow metabolizer)?
